Your first visit, step by step

Book your visit
Book online or call (818) 340-3062. If you're in pain, tell us when you call so we can see you as soon as possible.

Fill in your forms
Print your new patient forms and bring them with you, or arrive 15 minutes early and fill them in at the office.

Bring your ID and insurance card
Share your plan details ahead of time and we'll check your coverage before your visit, so there are no surprises.

Your exam
A thorough exam with Dr. Ahdout and any X-rays you need.

A clear plan
An honest conversation about what we find, your options and the costs. No pressure.
Insurance and payment
Payment is due at the time of treatment. We'll explain the cost of any treatment before it starts.

Yes. Smiles Creator welcomes new patients. Book online or call (818) 340-3062 and we'll find a time that works for you.
Bring a photo ID, your dental insurance card, your completed new patient forms and a list of any medications you take. If you don't have the forms, arrive 15 minutes early and fill them in at the office.
Smiles Creator accepts most PPO dental plans, HMO dental plans and Medi-Cal (Denti-Cal). Call with your plan details and we'll check your coverage before your visit.
Yes. Smiles Creator offers dental payment plans through CareCredit, Cherry and Sunbit, subject to approval, so you can start treatment now and spread the cost over time. We also accept cash and major credit cards.
Your first dental visit includes a thorough exam, any X-rays you need, and an honest conversation about what we find. You'll leave with a clear plan and no pressure.
Please give at least 24 hours' notice if you need to cancel or change an appointment. Appointments missed or cancelled with less notice incur a $75 charge per half hour, as explained in our financial policy.
Yes. Dr. Ahdout speaks English, Spanish and Farsi, and the team speaks English and Spanish.
